I'm so happy to see that so many of you have already done it! I need to gather minimum 30 answers for the statistics, otherwise the teachers won't accept it. If you guys can, please share it to your friends aswell :)

Done, but if you'll accept my critique;

You ask a lot of questions early on that don't seem to relate to the core of your form, and I found myself answering 'Other' to a lot of them too, in the future you may want to include more variety in the possible responses you give.

I wish you luck with your study, I hope the results are within your expectations.

 

Of course I accept critique. I will pass the feedback to her. It's the first time for us to make this kind of research. I can say that when we go to the statistics and conclusion, it all must sum up to atleast over 20 pages.
